照顧一位肺癌患者感染海洋弧菌併發壞死性筋膜炎之護理經驗

本文描述一名肺癌個案在家處理魚肉不慎被魚刺扎到左手第三指，因而感染海洋弧菌合併壞死性筋膜炎之護理經驗。照護期間為2017年7月26日至8月3日，筆者以Gordon十一項健康功能性評估系統為架構，透過直接觀察、身體評估、會談、實際照護與家庭互動等方式進行生理、心理與社會層面之整體性評估，確認個案有皮膚完整性受損、自我照顧能力不足以及焦慮等健康問題，在護理過程與個案和家屬建立良好治療性關係，依個案需求與個案、家屬以及醫療團隊共同擬定整體照護計畫，提供疾病照顧資訊與傷口換藥技巧並回覆示教、個別性的飲食攝取指導，進而提升個案自我照顧能力等護理措施。出院後經由電訪方式關心個案返家後自我照顧能力及疾病癒後。盼能將此疾病照護納入在職教育，藉由跨團隊照護進行個案討論以解決身心需求及情感支持，提升護理同仁的專業領域與照護品質，期望藉此護理經驗能提供護理人員未來臨床照護上之參考。

This article describes the nursing experience in taking care of a lung cancer patient whose finger was pricked by raw fish bone which leaded to necrotizing fasciitis infected by Vibrio vulnificus. The nursing care period was from July 26 to August 3, 2017. The Gordon's 11 health functional assessment systems were used as a framework for date collection by direct observation, physical assessment, interviewing, practical care, and family interactions. Overall, the case was involved impaired skin integrity, lack of self- care abilities, and anxiety and other health problems. During the nursing care, a good therapeutic relationship was established to health care professionals, the case and the family members. The decision making which based on the case need was decided by the case, the family and the medical team. The author formulated an overall care plan, provided disease caring information, skills for wound care and responded for teaching and individual dietary intake instructions for improving case self-care abilities and other nursing measures. After discharging, via phone call interviewing patient's situation of home care would be monitored. And this nursing experience can be a model for future guide for this kind of patient.
