運用希望建立技巧於一位胃癌末期無望感患者之護理經驗
Nursing Experience in Applying ＂Hope＂ on the feeling of hopelessness of a patient with end-stage gastric cancer
摘要
本文旨在探討運用希望建立技巧於一位胃癌末期無望感患者之希望重建護理經驗。護理期間為2016年1月3日至2016年1月21日，個案因無法接受胃癌快速惡化，對治療與未來感到挫折，藉由觀察、會談及身體評估收集資料，確立個案健康問題：生理上在住院期間有骨頭疼痛、食慾降低、睡眠障礙與全身虛弱之健康問題，亦出現被動、少語、情緒低落、情感反應淡漠等心理負向行為、以及靈性層面之無望感。筆者除給予個別性護理措施，改善其生理不適外，亦透過良好的護病關係、提供支持及病友經驗分享、確認威脅希望的原因、協助增進其自我照顧能力與自我控制感、讓家人或朋友參與照顧計劃、尋求信仰的靈性支持等希望建立技巧之運用，使個案對治療擁有正向態度與情緒，改善和他人的互動，幫助其調適疾病過程的變化，增強並重建個案希望以堅強面對未來之生活。

The purpose of this article was to utilize skills to assist a patient with end-stage gastric cancer to restore his feeling of hopelessness. During the period of 3rd ~ 21th January in 2016, data collection methods such as observation, conversation and physical assessment skills were used to gather information and identify health problems which included bone in pain, poor appetite, sleep disturbance, general weakness, depression, apathy and feeling of hopelessness . Nursing care plan was individualized and aimed to improve physical discomfort, minimize negative emotional responses, and to restore the feeling of hope. It was necessary to identify relevant sources and influencing factors of feeling of hopelessness before any interventions that were given to help enhance their self-care skills, improve a sense of self-control, and strengthen spiritual support with significant family members or friends who were involved in the care plan. The caregiving experience would shed some light to health care providers when caring for end-stage gastric cancer patients who encounter spiritual problems in the end of their lives. We hope that this article can raise clinical nursing staff's attention to the issue of feeling of hopelessness in the hope of establishing important skills. Through education and training, multimedia nursing education and problem-oriented learning can improve the quality of life among patients with end-stage gastric cancer.
