病人安全文化與藥物安全

Safety Culture and Medication Safety

摘要


醫療照護品質的基礎在於病人安全,病人安全的目的是讓病人免於因為醫療照護過程中的意外而導致不必要的傷害。國內外,病患安全已為各大醫療院所重視的課題,為讓病人安全觀念能深植於整個醫療體系,病人安全文化的四項特質:通報文化、公平文化、不責備文化及學習文化落實於實務中是必要的。安全文化建立的過程可由醫院行政管理制度、病人安全體系、醫護人員對於醫療疏失的信念和態度上著力;藉由跨部門、跨專業的合作模式,塑造一個以病人安全文化的照護環境,進而杜絕系統性或人為性的藥物疏失,確保藥物安全作業。

Patient safety is essential for medical quality. The goal of patient safety is patients avoid from unnecessary injuries while receiving healthcare service. Patient safety has become a national-wide issue. In order to ensure patient safety culture in healthcare organizations, there are four elements such as reporting culture, just culture, blame-free culture, and learning should be emphasized. Thus, it is possible to input these cultures to administration system and adverse event management, and to cultivate healthcare professionals, belief and attitude with medical error and patient safety concept. To promote patient safety oriented working environment and interdisciplinary collaboration is the way to avoid medication errors caused by system or human factors.
