黃疸新生兒照光治療照護模式之改善方案

Developmental Care of Newborns with Hyperbilirubinemia Undergoing Phototherapy

摘要


本專案旨在提升黃疸新生兒在接受照光治療時的照護品質。經現況查檢及分析,發現傳統照光的護理方式易有下列問題:照光及扎針時,新生兒生理徵象較正常新生兒不穩定;睡眠時間易受干擾而減少;親子關係之建立受影響;母親繼續哺餵母乳之意願易改變。經由現況分析及文獻查證後,決定運用發展性照護模式來改善現行照光治療的照護模式。發展性照護措施施行結果發現,黃疸新生兒於扎針時心跳由154降至142次/分、呼吸由56降至43次/分、血氧飽和濃度由95%上升至98%;扎針時哭泣時間由5.4分鐘降低為2.3分鐘;睡眠時間由16.6增加至18.4小時;親子連結方面由42.8%上升至70%;親子互動方面由28.6%上升至60%;母乳哺餵改變率由71%降低至45%,證實發展性照護措施可提升新生兒黃疸照光的照護品質。

Preliminary survey results indicated that traditional phototherapy for newborns with hyperbilirubinemia is associated with unstable vital signs, reduced sleeping time, poor parent-child bonding, and decreased breastfeeding rate. Post-implementation of newborn developmental care showed improvement in the following areas: (1) heart rate during needlestick reduced from 154 to 142 per minute, (2) cry time during needlestick reduced from 5.4 to 2.3 minutes, (3) sleep time increased from 16.6 to 18.4 hours per day, (4) bonding increased from 42.8% to 70%, parental-newborn interaction increased from 28.6% to 60%, and breastfeeding failure rate reduced from 71% to 45%. In summary, newborn development care improves quality of newborns with jaundice under phototherapy.
