裝置永久性心律調節器病人之護理經驗
Nursing Experience of Permanent Pacemaker Implantation
摘要
本文主要探討一位70歲老年人在緊急接受心律調節器植入術前後歷程之護理經驗。筆者於95年8月14日至95年8月16日照護個案期間，運用Gordon十一項功能性健康型態為評估工具，藉由直接護理、觀察、會談及身體評估等方式收集資料，確立個案的護理問題有：心輸出量減少、知識缺失、焦慮等，並針對個案擬定個別性護理計畫。筆者在護理過程中，對於個案因對疾病不清楚，而誤解醫療處置，及對於術後自我照護的不確定感而感到焦慮，經由術前與個案及家屬詳細溝通，使其瞭解心律調節器之重要性，並於術後衛教返家後日常生活注意事項，與家屬共同提供心理支持及護理指導，為病患解決疑惑，減輕病患裝置心律調節器後的心理負荷，以增加生活品質，因此將此護理經驗與大家分享，以提供護理人員日後照護此類病患之參考。

This paper discusses the nursing care of a 70-year-old patient who received emergency cardiac pacemaker implantation. Nursing assessment used Gordon 11 functional health patterns through direct care, observation and physical examinations. An individualized nursing care plan outlined the following health problems: (1) reduced cardiac output, (2) lack of knowledge, and (3) increased anxiety. The patient's anxiety related to lack of knowledge of both medical management and post-procedure self-care. Health teaching included the purpose and functions of cardiac pacemaker, post-procedure home care, and psychological support. The interventions demonstrated success in easing the patient's doubts about cardiac pacemaker implantation and improving post-procedure quality of life.
