Guillain-Barre'症候群合併呼吸衰竭之加護經驗
Nursing Experience of a Patient with Guillain-Barre' Syndrome Complicated with Respiratory Failure
摘要
本篇探討一位年輕個案罹患Guillain-Barre'症候群神經病變的護理經過。筆者在照護期間2010年4月16日至4月27日，經由使用注音符號拼音會談，克服個案因疾病及氣管插管導致無法以言語溝通的障礙，藉此建立良好護病關係，並於治療改善後以筆談及會談方式，運用Gordon十一項健康功能評估，確立有呼吸道清除功能失效、潛在危險性皮膚完整性受損及焦慮之健康問題，護理過程中協助維持個案肺部氧氣交換，發展有效咳嗽能力，提供個案及家屬對疾病有正確的認知，並預防身體固定不動導致可能的皮膚完整之受損，協助度過住院治療期間種種生理及心理上的不適，並在治療及照護中逐漸恢復原有的身體活動功能。

This article describes the nursing care experience of an young adult with respiratory failure who was diagnosed with Guillain-Barre' syndrome. Data collected during interview used phonetic transcription, writing and dictation. Based on Gordon's 11-item functional health assessment, patient's health problems were: (1) ineffective airway clearance, (2) risk for impaired skin integrity and (3) anxiety. Nursing interventions included maintenance of pulmonary oxygenation, effective cough, providing patient teaching on Guillain-Barre' syndrome disease process, prevention of pressure ulcer, and assisting the patient to adjust to hospitalization both physically and psychologically.
