摘要

本文就英國與美國安寧居家療護之緣起及相關制度概略介紹，並以台灣安寧居家療護之發展及制度分享照護經驗，描述在面對社會變遷下台灣之因應方法。除參照英、美體系方式與經驗，更期望針對台灣現有制度能有所創新，使安寧居家療護甚至長期照護體系，能發展出符合國情的安寧居家療護模式，造福病人及家屬。

This article describes the origin and systems of home-based hospice palliative care in Europe and America, and the evolution, establishes the system and experience in Taiwan, and the needed adjustments due to societal changes. Besides referencing the systems in Europe and America, the author calls for innovation to develop a new model of home-based hospice palliative care that may include long-term care which is more suitable to Chinese culture and society in Taiwan.
