糖尿病行胃繞道手術後併發合併症個案之護理經驗
The Nursing Care Experience of a Diabetic Patient with Post Roux-en-Y Gastric Bypass Surgery Complications
摘要
本文在敘述一位接受胃繞道手術後產生合併症的肥胖個案。胃繞道手術對病態性肥胖及糖尿病治療有顯著的成效，然而手術並非無風險或相關合併症產生。肥胖患者若對手術結果不如預期，除沮喪、失望外，有可能引發出憂鬱情緒。筆者在2011年8月3日至2011年8月11日照護期間，經由與個案深入會談、觀察、收集病歷資料及整理分析後，發現個案主要有：「現存性感染」、「健康維護能力改變」及「身體心像改變」之問題。護理過程中與病人建立良好護病關係，經由協助個案了解減重手術知識、引導個案參與、學習自我照顧及增加個案信心，成功的協助個案出院返家。希望藉此個案的照護經驗，幫助護理同仁認識減重手術及相關合併症護理知識。

This article presents a case of a morbidly obese patient who experienced altered body image post- bariatric surgery complications. Roux-en-Y gastric bypass surgery is effective in the treatment of morbid obesity and diabetes mellitus. Frustration and disappointment from less than expected weight loss may lead to depression. Over eight days in-depth interviews, observations and review of the medical record, the author identified the subject's problems as (1) infection, (2) impaired health-maintaining ability and (3) disturbed body image. Nursing interventions included building a good relationship with the patient, teaching on the weight-reduction surgery, involving the patient in self-care activities, and boosting the patient's self-confidence. These key activities successfully prepared the patient for discharge from hospital to home.
