背景:2015年八仙塵暴救治時單位發生二例病患因整形外科手術護理師於微粒植皮手術過程中操作錯誤造成移植皮膚無法使用事件,而後調查發現於此術式中整形外科手術護理師操作完整率僅46.0%。分析造成問題之主因為:整形外科手術護理師操作技能訓練不足、教學策略不完善、缺乏標準作業流程、器械不足。目的:改善整形外科手術護理師微粒植皮手術操作品質。解決方案:專案執行期間自2016年4月6日至2017年12月31日,規劃相關措施如單位在職教育之實體及線上課程、訂定標準作業流程、增訂稽核項目及評值表、跨團隊擬真訓練、拍攝示範影片等。結果:整形外科手術護理師微粒植皮手術技能操作完整率由46.0%提升至97.8%,認知得分正確率由66.9%提升至97.7%。結論:此專案能有效提升微粒植皮手術護理品質及增進手術護理師專業知能。
Background: In the 2015 Water Park blast incidence, there were two patients who experienced the skin graft failure due to errors made by operating room nurses in the process of MEEK micrografting. An inves¬tigation revealed that the completeness rate of MEEK micrografting for plastic operating room nurses was only 46.0%. The main causes were lacking in operating skills, training strategies, standard operating proce¬dures and adequacy in equipment. Purpose: To improve the quality of plastic operating room nurses' skills on MEEK micrografting. Resolution: Implemented improvement strategies between April 6, 2016 and De¬cember 31, 2017. The project team instituted online in-service education courses, established standard oper¬ating procedures, updated audit items and evaluation sheets, held cross-team simulation training and filming demonstration videos. Results: The completeness rate of MEEK micrografting by plastic operating nurses improved from 46.0% to 97.8%, and the correct rate of cognitive scores increased from 66.9% to 97.7%. Conclusion: This project effectively improved the quality of MEEK micrografting operating skills and the surgical professional knowledge for plastic operating nurses.
