提升兒童加護病房個案報告繳交率之改善專案

撰寫個案報告是目前護理人員能力進階的重要依據，也是護理專業能力評估方式，更會影響整體醫療照護品質。本專案旨在提升兒童加護病房個案報告繳交率，資料顯示2017年7月及2018年1月單位個案報告繳交率僅達20.0%，低於本院繳交率38.2%，經分析發現問題：無同儕勉勵、自覺無時間書寫報告、無法掌握報告書寫技巧、自覺寫報告困難、單位缺乏輔導機制、個案報告相關課程少、單位無書寫風氣等。經專案小組建立N3報告撰寫LINE小組、舉辦個案報告撰寫課程、派專人輔導、安排圖書館資料檢索教育訓練課程、舉辦撰寫成功人員經驗分享等措施。結果顯示個案報告繳交率從20.0%提升至34.4%，顯示本專案之改善策略成效卓著。

Writing case report is an important reference for nursing staff career advancement. It assesses the nursing profession and may affect overall quality of medical care. Data indicated that the case report submission rate in a pediatric intensive care unit (PICU) between July 2017 and January 2018 was 20.0%, below the average rate of hospital of 38.2%. The purpose of this project is to improve the PICU staff nurse case report submission rate. Factors related this low rate included lack of peer encouragement, lack of time, lack of writing skills, afraid to write report, lack of counseling system in the unit, lack of learning courses, and lack of proper writing atmosphere in the unit. The project team established the N3 report writing LINE group, involved writing report experts to teach and to share their experience, and provided courses on how to conduct literature review. The case report delivery rate increased from 20.0% to 34.4%, indicating that the improvement strategies were effective.
