支架置放術之冠狀動脈三維數值分析

摘要


在臨床上血管支架置放術已被廣泛使用來治療冠狀動脈疾病,但實行支架置放術後的病患,血管還會再窄化。部分研究指出當血液流過支架區域時,因?支架的幾何結構,致使血流在許多凹槽區域形成渦流,造成血塊在此處停留而形成血栓,使血管再度窄化。本研究探討支架置入血管的情形,並嘗試將血液的流場效應考慮在分析模型中,透過加入氣球膨脹及血管壁的回壓力量計算出支架的變形,然後模擬血流流過含有支架的區域,分析血管壁和支架承受的壓力及流體動力學的效應。這樣一來,就可以透過支架變形後的流場數值模擬,較真實的探討臨床上實施血管支架置放術後血液的流動行?,以瞭解支架外形結構對血液流場的影響。

In this research, we include the vascular flow effect in the study of the coronary artery stent. The deformation of the stent due to the balloon dilatation and the reaction of the artery wall is taken into account to simulate the vascular flow field around it and to observe the pressure distribution against the stent and artery wall. The deformation of the stent resulted from the deformation of the blood vessel may be found out. Through the post-deformation numerical simulation of the flow field passing the stent, we are able to study a more realistic vascular flow behavior after the coronary by stent implantation as well as its influence to the vascular flow field.
