摘要

完整的出院計劃在骨科病房是重要的，骨科病患需要有完整的出院計劃以協助面對後續在活動上的限制、預防跌倒以及返家後等問題。提供病人及家屬完整的出院計劃，可確保其持續性照護及減少返家後的焦慮。本專案的目的爲整合骨科病房出院計劃的一致性。在現況分析中發現本病房無明確的書面出院計劃標凖，護理人員依個人經驗處理病患的出院計劃，而不完整的出院計劃會影響病患的預後，及造成病患對出院率備的滿意度偏低。本專案藉由建立出院計劃流程、對護理人員進行與出院計劃議題相關的在職教育、及持續追蹤評估病患對出院計劃認知及滿意率。結果顯示護理人員對出院計劃認知由專案前的74.3%，在專案執行後增加爲100%；病患對出院計劃的認知由專案前的71.3%增加至95%；病患的滿意率由專案前的76.5%增加到專案後87.5%。此專案建議有效的出院計劃是能提升骨科病患出院後的處理認知及增加對出院服務滿意率。

Effective discharge planning is critical in orthopedic wards. Orthopedic patients need to have integrated discharge planning to deal with issues of immobilization, fall prevention and posthospitalization vulnerability. Providing effective discharge planning for patients and families is important to ensure continuity of care and freedom from worrying at home. However, there is no standard discharge plans for nurses in orthopedic wards to refer to. Therefore, to help our nurses to write better and more effective discharge plans, this project established a protocol of discharge planning, provided in-service training for nursing staff, and administered assessment of patients' satisfaction and nurses' knowledge concerning discharge planning. Results revealed that nurses' knowledge related to discharge planning and current discharge services increased from 74.3% to 100% and that overall patient knowledge increased from 71.3% to 95%. Patients' satisfaction rate also increased from the 76.5% to 87.5%. This study suggested that effective discharge planning could help orthopedic patients improve their knowledge about post-hospitalization care and increase their satisfaction concerning discharge service.
