摘要

本文描述一對夫婦，其太太於妊娠20週，經羊膜穿刺術檢查結果胎兒爲唐氏症而終止妊娠。筆者於97年1月19日至97年l月21日擔任個案的主護護士，出院後並進行二次夫婦的面訪，在護理過程中以田野研究法收集語言、非語言的行爲資料，寫成五篇行爲過程實錄，並經由內容分析法分析，資料分析歸類出夫婦在面臨得知胎兒染色體異常至接受終止妊娠，經歷了「難耐胎兒染色體異常訊息的宣佈」、「保留或拾棄的抉譯」、「引產的煎熬」、「設法釋疑消解心中的芥蒂」，希望藉由此夫婦親身經歷受的呈現，能讓護理人員對此經驗有更深層的了解，並作爲護理人員在協助夫婦面對胎兒染色體異常接受終止妊娠的參考，進而能提供以家庭爲中心的護理照護。

This report describes a married couple in which the wife screened positive for a fetal chromosomal abnormality by amniocentesis at 20 weeks gestation. Based on the results, a decision was made to terminate the pregnancy. The author was the primary nurse for the patient from 19-21 January 2008. Following hospital discharge, two interviews were conducted with the couple. During the nursing process, a field method was used to collect verbal and non-verbal behavioral data. Subsequently, five process recordings were documented. By applying the content analysis method, the data for the experience of the couple were classified into the experience for termination following the diagnosis of the chromosomal abnormality as follows: ”unbearable fetal chromosome abnormality the message declare,” ”deciding on whether to continue or terminate the pregnancy,” ”enduring the pain of induction,” and ”trying the explanation eliminates the unpleasantness.” By presenting the personal experience of the couple, nurses will have an in-depth understanding of the termination experience. Furthermore, this study may provide reference for nurses to assist married couples in decision-making regarding termination due to a fetal chromosomal abnormality. Therefore, family-centered nursing care can be administered.
