摘要

本文旨在描述一位家族腺瘤息肉症患者，執行回復性直腸截除術及迴腸造口術， 所造成的生理及心理衝擊的護理經驗。護理期間自2009年7月6日至2009年7月19日。在照護期間運用Gordon十一項健康功能型態評估，確立個案有：「焦慮」、「急性疼痛」、「對迴腸造口照護知識缺失」、「潛在危險性感染」之健康問題。於護理過程中，筆者藉由陪伴與傾聽、鼓勵個案說出心裏的感受及針對其擔心之事，予詳細解釋及澄清疑惑之處，以降低焦慮之情緒，另外給予緩解疼痛、造瘻口照護注意事項、以及預防感染的相關護理處置，提供個別性的護理指導；盼藉此護理經驗能提供護理人員作為日後在照顧類似病患之參考。

This case report describes the nursing care experience in nursing a patient with familial adenomatous polyposis (FAP) with restorative proctectomy and ileostomy. The nursing care duration was from July 6 to July 19, 2009. By applying Gorden's 11-item functional health assessment, the patient's health problems were identified as anxiety, acute pain, deficient knowledge of ilealostomy care, and high-risk infection. The author encouraged the patient to explain her feelings and concerns in order to solve the patient's nursing problems. In addition, the author provided the patient with explanations for and clarified her doubts about reducing anxiety. The nursing interventions include anxiety reduction, pain alleviation, ilealostomy care, and infection prevention. The nursing interventions described in this study serve as a reference for caring for FAP patients in the future.
