摘要

伴隨人口老化趨勢，我國需長期照顧人口快速增加，為回應長者社區照顧之需求，長期照顧政策之推展以居家、社區式照顧為趨勢，佈建普及可近之照顧服務資源。另為強化社區整體照顧模式，建置連續多元照顧體系，衛生福利部於104年度試辦多元照顧中心（小規模多機能）服務，推動以社區為中心、實踐連續性照顧、打造個人化服務之照顧計畫，滿足長者照顧需求，落實全人照顧之政策目標。多元照顧中心服務係屬整合性服務模式，以日間照顧中心為基礎，擴充辦理居家服務、臨時住宿等多元服務，透過結合在地照顧人力、社區照顧資源，提供多元、彈性且連續之照顧服務，以長者為中心佈建資源網絡，促進長者在社區中生活的可能，並在照顧中延續長者家庭生活以及維持與社區之連結，未來賡續評估試辦成效，鼓勵縣市政府依福利資源配置普及設立，並積極開發、統合社區照顧資源，針對社區長者個別化照顧需求，提供適足之多元服務，以完備社區整體照顧服務輸送體系。

Population aging has caused the need for long-term care in Taiwan to increase rapidly. To meet the community care needs of older adults, a long-term care policy promoting home care and community care was established to provide universal and accessible care. In addition, in order to reinforce comprehensive care services and build a continuous and multifunctional system, the Ministry of Health and Welfare started to promote the small-scale and multifunctional service in 2015. This project provides communitybased, continuous, and personalized care service to meet older adults' needs and achieve the holistic care policy goals. The small-scale and multifunctional service operates on an integrated service model which mainly provides day care along with various services such as home care and temporary accommodation. The service incorporated the local workforce and community care resources to provide various, flexible, and continuous care. An older adult-oriented resource network is being built to help older adults live in their community, continue family life, and connect to the community. In the future, the effects of the smallscale and multifunctional service will be evaluated. Local governments are encouraged to establish multifunctional service in accordance with their welfare resources and develop integrated community care resources in order to provide appropriate individualized care and create a comprehensive community care system.
