懷孕期接受兩次腹部手術之初產婦護理經驗－病例報告

大多數婦女在懷孕開始都期許有正常的懷孕過程，但少部份女性並非如此幸運，其懷孕過程充滿著考驗與危機。筆者於產前門診接觸一位初產婦，因腹部膿瘍而接受兩次手術，即個案懷孕32週至產後兩週，透過觀察與會談，了解並紀錄個案的表現，確立其護理問題有：1.對胎兒健康的不確定感/與疾病與手術的危險性有關；2.喪失控制感/與自我照顧能力缺失（沐浴）有關；3.渴望支持/與懷孕過程不順利而先生忙於工作有關。在護理過程中，傾聽她的喜怒哀樂，分享她為人母的喜悅，更分擔孕程不順對她造成的挫敗感，最後終於使其充滿信心的迎接新生命。個案先生亦在此過程中，學習到生產應是夫妻共同面對與分享的經驗。希望藉此報告呈現高危險妊娠產婦的心路歷程，期望臨床醫療團隊給予此類產婦更多支持，並促使護理專業能多用心，同時在照顧婦女過程中，能擴展其護理專業獨特性功能及角色。

Most women expected to have normal pregnant process when starting pregnancy and also could be expected and achieved for pregnancy, delivery, and becoming a mother successfully. Some women were not so lucky, however, they faced the challenges and crisis during pregnant process. In this case report, the researcher interviewed a primipara received abdominal operation two times at out-patient department due to abdominal abscess between the 32 weeks of gestation and two weeks after delivery. From the performance of maternal tasks of the case, the nursing problems through interview and observation were: 1. feeling of uncertainty/related to the disease and risk of operation. 2. feeling of lose controlled/the difficulty in self-care (washing oneself) independently. 3. aspired to support/related to dissatisfactory pregnant process and her husband's busy in work. From companion, listen attentively, empathy, and concern methods, the case could still keep confidence in pregnancy and welcome the newborn. The case's husband also shared the nursing process and learned that the couple should face and share the experience of childbirth with each other. We hope this nursing experience could provide more information to take care for the high risk pregnancy subjects and expand the role and function of nursing profession.
