運用羅氏理論協助一位未成年母親面對早產兒的護理照護：病例報告

本篇病例報告是描述協助個案（15歲未成年母親）學習照顧早產兒的過程。筆者於護理期間2006年7月14日至2006年8月20日擔任案子主護護士，並在此期間利用會客時觀察個案與案子的互動，以會談、查閱病歷紀錄、電訪等方式，收集案子及個案資料，運用羅氏適應模式進行個案評估。照護過程中歸納其主要護理問題有：潛在性危險父母、早產兒照護知識缺失、家庭運作過程改變等。針對上述問題，給予照護指導並採取漸進方式啟發個案學習意願，進而熟悉早產兒的照顧技巧，藉由依附關係的發展，提升母性行為及情感的表達，促進親子關係的建立，並由社工介入居中協調、溝通，改善個案應付家庭壓力，使個案能接受並扮演好一位母親的角色。希望藉此照護經驗與臨床同仁分享。

This case report describes nursing experience of helping a fifteen years immaturity mother face to a premature baby. The duration of nursing care was between July 14, 2006 and August 20, 2006. The methods of data collection included face-to-face interview, chart review, and telephone interview. The evaluation was based on the Roy adaptation model. The case's health problems included potential risk parents, lack of knowledge of premature baby's care, and the changed family's process. For these problems, the author designed a nursing plan included: 1) Make gradual progress to inspire case's learning aspiration and then practice the premature baby's care well. 2)Based on the development of parent-infant attachment, increased the maternal behavior and emotional express, then established the mother-infant relationship. 3)To coordinate and communicate by the social worker and improve the management of the family pressure, then the case could accept and play the role of mother well. This case report was expected to share the nursing experience and provide reference to clinical nursing specialists for further caring similar cases.
