Intrauterine fetal brain death is a rare cause of a fixed fetal heart rate pattern, without variability, accelerations or deceleration. There was no reaction to vibroacoustic stimulation test. Obstetric ultrasonography revealed an atonic fetus with no breathing or body movements. Percutaneous umbilical blood or fetal scalp sampling showed a normal pH value. In this way fetal distress can be excluded and unnecessary operative intervention can be avoided.
