摘要

目的：藉分析分別以risperidone及olanzapine治療者，觀察兩藥品療效與安全性的差異。方法：數據取自南部某醫學中心健保申報資料。主診斷須符合國際疾病代碼（ICD-9-CM）：295XX。統計出只以risperidone或olanzapine治療精神分裂症的患者。藉分析每次平均總藥費、每日平均處方劑量與每日定義劑量比值觀察費用。藉分析住院天數及再住院率觀察有效性。藉併用藥品觀察安全性。結果：Risperidone每日平均處方劑量顯然較低。每日定義劑量比值在兩組間不具顯著差異。Risperidone每次住院平均總藥費顯然較低。Olanzapine顯示出院6個月內的再住院率較risperidone少，至於出院後12個月的再住院率，兩組則沒有統計學上的顯著差異。兩組住院天數同樣沒有顯著差異。併用藥品方面，顯示risperidone只有併用trihexy-phenidyl機率較高，但olanzapine併用其他藥品的情況卻遠遠超過risperidone。結論：本研究發現在住院天數及藥費方面，risperidone是較好的選擇。而出院後6個月內的再住院率及併用trihexyphenidyl的情況，則傾向於選擇olanzapine。

Objective: To compare the effectiveness and safety with patients who take risperidone or olanzapine only.Methods: 147 inpatients with single therapy; 119 taken with risperidone and 28 accepted by olanzapine, from July 1 2005 to June 30 2006 had been retrospectively reviewed and continually follow-up after discharged from the hospital, particularly after two periods of 6-month and 12-month. Chi-square, ANOVA and independent t-test are used to evaluate their outcomes.Results: Olanzapine had higher medicine costs and effectiveness and efficiency. Patients taken with risperidone had shorter admission days.Conclusions: The study found that an aspect of hospital days and medicine cost, risperidone is a better choice. However, considering readmission rates of six months after discharge and combination-use trihexyphenidyl is better to choose olanzapine.
