Infective endocarditis of the tricuspid valve is a common complication of injection drug use. However, the relapsed tricuspid valve endocarditis in drug addict after successful self-withdrawal was uncommon, and tricuspid prosthetic endocarditis was seldom described in such patients. A previous drug addict developed a relapsed tricuspid bioprosthetic endocarditis four years after tricuspid valve replacement. He was managed conservatively with antibacterial treatment and recovered favorably. Conservative management of tricuspid prosthetic endocarditis may be preferable in the injection drug users, especially in those with a successful self-withdrawal from drugs.
