Venous thromboembolism (VTE) is common complication following multiple trauma and orthopedic surgery, especially lower limb arthroplasty. In fact, incidence of VTE following fracture of the long bones has been reported to be between 1% and 2%. Risks of VTE following operative treatment of ankle fracture include deep vein thrombosis and pulmonary embolism (PE). Here, we present a rare case of ankle fracture complicated by PE, which highlights the difficulty in detecting potential risks of thromboembolic events due to their rare and delayed occurrence in foot and ankle operations. We also review the published data regarding the prevalence of thrombosis, risk factors, and methods of prophylaxis in surgical procedures of the lower extremity.
