一位顏面灼傷行重建整形術個案的護理經驗

The Nursing Experience of a Facial Burned Injury Patient, Who Undergoes Plastic Surgery

摘要

本篇報告是探討一位39歲女性，遭硫酸灼傷右側顏面、肢體等，先後經過十次住院、手術，此次為施行右耳重建整形手術而入院之個案。護理期間2003.06.12至2003.06.20，採Gorden十一項功能性健康型態評估，以臨床照護觀察及與個案會談等方式收集資料，確立個案的主要健康問題為「焦慮」、「睡眠型態改變」、「身體心像紊亂」、「家庭運作過程改變」。藉由提供相關訊息、介紹成功案例、運用傾聽及溝通技巧等強化個案正向身體心像及價值，使個案察覺家人間互動的問題及討論改善家人間關係的方法等，使個案能以正向的態度，面對漫長的顏面灼傷後的身心癒合歷程。

We report a case of a 39-year-old female patient who was a victim of facial chemical burn with sulfuric acid from the assault by an offender. She had been hospitalized 10 times for reconstructive surgery before. This time, she was admitted again for right ear otoplasty. In the frame of the Gorden's 11-item functional health evaluation, after data collection and analysis, we found the main themes of her health problems are anxiety, sleep disturbance, body self-image disorder and change of family interaction. During the period of our nursing care, we alleviated or soothed her problems by giving relevant information including information about those successful cases, providing wound care cradle, applying skillful techniques of listening and communication, supporting her body image and value positively, making sense of her family dynamics and discussing the strategies of improving her family interaction. We positively support her altitude so that she had the courage to face the long process of physical and psychological healing.
