摘要

本院接受國民健康局獎助，辦理門診子宮頸抹片電腦主動提示系統，於2007年7至9月間，經此系統完成子宮頸抹片受檢人數僅31人，受檢率為0.93%，未達預期目標，且較當年度獲選為績優醫院之受檢率48%偏低許多，分析受檢率低之原因有：（1）跟診人員未提醒；（2）激勵措施不足；（3）檢查服對病人隱私性保護不佳；（4）病人因素。本專案目的在提昇門診子宮頸抹片主動提示系統轉介受檢率，解決方案包括：（1）舉辦跟診人員對子宮頸抹片之認知教育課程；（2）修訂轉介子宮頸抹片受檢之獎勵辦法；（3）縮短等候抹片篩檢時間；（4）制定轉介抹片篩檢之標準用語；（5）提昇受檢環境隱密性。方案實施後，統計3個月轉介受檢人數共1,542人，受檢率為24.4%，上升了23.4%，達本專案目的。

This project was subsidized by the Bureau of Health Promotion to establish an OPD (out patient department) computerized on-line prompting system. Results from a preliminary study showed that the screening rate, during the first three months (from July to September, 2007), was only 0.93%. The rate was far behind the annual benchmark of the hospital (48%). Low screening rate was related to four factors: (1) OPD staffs were failed to remind patients; (2) insufficient incentives for OPD staffs; (3) patient's gown can not protect patient's privacy; (4) patient's personal characteristics. The purpose of this project was to raise OPD Pap smear screening rate through computerized on-line prompting system. Five solutions were used in this project: (1) educational program of Pap smear was implemented for OPD staffs; (2) awarded staffs if they successfully referred patients; (3) streamlined the OPD Pap smear process; (4) standardized dialogues in patients' persuading process; (5) improved environment privacy. During the first 3 months after this project, the number of successfully referred patients for Pap smear was 1,542 (screening rate was 24.4%). By this project, the goal of increasing OPD Pap smear screening rate was achieved.
