康復之家精神疾患者之再住院影響因素

A study on impact factors of patient readmission in halfway houses

摘要


研究目的在於探討接受住宿型精神復健機構服務的精神疾患者,影響其後續再住院的因素。本研究分析北部某一間康復之家的結案住民,採回溯性研究,取樣時間自2012年8月至2017年12月的結案資料,累計有105位樣本數,依結案原因分成精神科住院組(N=46)與非精神科住院組(N=59),資料蒐集方式為個案服務紀錄。研究結果發現性別、居住型態、障礙程度、福利補助及家庭支持當中的情感關懷、返家協助、帶日用品、參與復健計畫為精神疾患者的再住院影響因素;而家庭能否提供費用負擔(p<0.001)、提供日用品(p<0.05)、參與復健計畫(p<0.001)這三項支持可預測精神疾患者的再住院風險。研究建議應關注社區精神復健機構精神疾患者的住院影響因子,思考如何協助家庭提供適度的支持以降低社區精神疾患者的住院風險及家屬的照顧負荷。

This is a study to explore factors contributing to psychiatric patient readmission in halfway houses. This study was based on fieldwork conducted from August 2012 to December 2017 in a halfway house in Taoyuan, Taiwan. Research participants (n=105) were divided into two groups: hospital group(n=46) and non-hospital group(n=59). It evaluates performance and inferential statistical factors which include their demographic, clinical, psychiatric and rehabilitation services, welfare subsidy and family support variables by using SPSS 20.0. Those impact factors include sex, living arrangement, level of disabilities, welfare subsidy, family support (focusing on emotional support), and arrangements after discharge, accessibility of daily necessities, and level of participation in the rehabilitation program. Cox regression model showed family support with expense sharing (p<0.001), accessibility of daily necessities (p<0.05), and level of participation in the rehabilitation program (p<0.001) are the main factors which contribute significantly to psychiatric hospitalization. Community rehabilitation care should focus on assessment of the impact factors, and providing appropriate support to family in need. This could strengthen family relationships, avoid care burden and decrease further hospitalization.
