Cardiac Arrest in Patient Who Underwent Transoral Endoscopic Thyroidectomy : Case Report
經口腔內視鏡甲狀腺切除術中心臟停止之個案報告
摘要
Transoral Endoscopic thyroidectomy has gained its interest and popularity modern-day among all kinds of health care services all over the world to work as ＂scarless operation＂ and ＂minimally invasive surgery＂ because of its cosmetic and aesthetic improvement. This is a 34-year-old female without any significant past medical history scheduled for transoral endoscopic thyroidectomy vestibular approach (TOETVA) surgery, during which the unexpectedly cardiac arrest occurred. By clinical pictures (e.g. dropped end tidal carbon dioxide (EtCO_2)) and progression, the carbon dioxide (CO_2) pulmonary embolism was highly suspected. We remind that clinician should not trap into the thinking that low CO_2 inflation pressure, which is reasonably safe during surgery, will not cause the CO_2 embolism.
