台灣南部中高齡居民健康體能因子、手部握力及其相關因子探討

Study on Health Fitness Factors, Grip Strength and Related Factors of Middle Age and Elderly Residents in Southern Taiwan

摘要


背景:近年來醫學界對「衰弱症候群」有大量的研究,普遍認為人類衰弱的指標包括:體重快速減輕、經常表示身體有疲倦感、手部握力變差、活動力減低以及動作變得緩慢等。其中「手部握力」的表現可以預測受試者的死亡率及失能狀態、手術後產生併發症的機率,以及患者留院的時間長短,是一個重要的綜合性生命力檢測指標。本研究在透過手部握力了解民眾的健康體能狀態,並進一步分析影響受試者手部握力表現的相關因子。方法:檢測項目包括:身體質量指數、骨密度、基礎代謝率、內臟脂肪、身體平衡力。受試者是台灣南部社區民眾,總計受試者共收錄382位男性(65.10±11.85歲);582位女性(63.80±11.52歲)。結果:研究結果發現,影響手部握力的因子為性別、年齡、基礎代謝率和靜態平衡力。其中,女性、年齡和手部握力呈現負相關;基礎代謝率和靜態平衡力則與手部握力呈現正相關。靜態平衡力對手部握力的預測力是本次研究的重要發現,可協助老人衰弱因子的篩選;同時呼籲在健康促進內容規劃上,應針對性別差異給予不同的建議。相較於女性受試者,男性手部握力的表現相對較弱,且個體間的差異較大,尤以中年男性握力最差。結論:影響社區居民手部握力的相關因子為性別、年齡、基礎代謝率和靜態平衡力。建議提早從中年期開始健康照護的規劃,並注意男性與女性不同的健康體能狀態與健康促進需求。

Background: Studies in recent years on frailty syndrome have accumulated substantial data about indicators of human aging, including unintentional weight loss, self-reported exhaustion, weakness (grip strength), slow walking speed, and low physical activity. Of these indicators, grip strength can be seen as a predictor of mortality, disability, complications, and increased length of hospital stay. Grip strength in fact provides a comprehensive overview of one's vitality. This study aims at understanding the physical health status of elderly residents in Southern Taiwan by examining grip strength performance and related factors. Method: Test items include: body mass index, bone density, basal metabolic rate, visceral fat, static and dynamic body balance. Participants incorporates 382 males (M=65.10±11.85 years old) and 582 females (M=63.80±11.52 years old) in Southern Taiwan. Result: The study shows that the main factors affecting grip strength performance are gender, age, basal metabolic rate, and static body balance. Female and age are negative correlation factors of grip strength performance, whereas Basal metabolic rate and static body balance are positive correlation factors. Further analysis identifies a high correlation between static body balance and grip strength. Compared to their female counterparts, male participants report weaker performance and greater individual differences in grip strength with middle-aged participants showing the worse performance. Conclusions: The study suggests that health and care planning should be initiated at the beginning of middle age with the different physical conditions and health needs of male and female duly recognized.
