An 85 year-old male was admitted due to chest tightness and abdomen discomfort lasting for about 2 months. No associated disease was confirmed in spite of the various physical and image examinations conducted during his hospitalization. After comprehensive geriatric assessment, mental state examination and psychiatric history taking, we found the patient troubled by depression and feelings of loneliness and helplessness and suspected that his somatic complaints might be atypical symptoms of geriatric depression. After treatment with antidepressants, the patient reported significant improvement in emotional symptoms and no longer complained about physical discomfort. This case report reminds clinicians to take heed of atypical presentation of elderly depression so as to facilitate early detection and intervention.
