使用智慧型手機建立年長者衰弱檢測系統:身體活動量與步行速度量測

Smartphone Application for Frailty Detection: Measurements of Physical Activity Counts and Walking Speed

摘要


目的:以智慧型手機為基礎,建立一衰弱檢測與早期介入之個人化隨身健康助理系統。方法:開發搭載於智慧型手機上的衰弱檢測與早期介入軟體,用以檢測自身衰弱的徵狀,及提供個人化的早期介入對策,並招募高齡長者參加檢測,使用相關性統計分析衰弱因子,驗證其檢測能力,再針對介入成效加以探討。結果:在22位受試長者中,屬於衰弱者與衰弱前期者各有一位,其餘均未出現衰弱徵象,且傳統檢測與手機檢測的結果並無二致。兩檢測方式對於身體活動量(r=0.747)與行走速度(r=0.800)的測試結果均呈現高度相關;然而,由於衰弱前期的收案數較少,僅能透過觀察得到本系統對於衰弱因子的改善具有潛力。結論:本研究建立的衰弱篩檢系統,可以取代傳統的人工測量或問卷調查,快速且準確地瞭解老年人的衰弱狀況。同時,電子化的量測,可提供量化的數據,定義衰弱的嚴重程度或是惡化速度,進而採行不同積極程度的介入策略,將有效避免醫療資源的浪費,妥善運用社會福利,降低整體社會成本的支出。

Purpose: To establish an individualized mobile health assistant manager for frailty detection and early intervention on smartphone application. Methods: A smartphone program was developed and elders were recruited for examination. The correlation analysis was performed for the screening ability and the effectiveness of intervention was discussed. Results: Twenty-two elders were participated and one was frail, while another was pre-frail. There were not any frailty signs among other 20 subjects. Both detection methods showed equally grouping results. Moreover, the correlation of physical activity counts and walking speed estimation between 2 methods demonstrated highly related (r = 0.747 & 0.800, respectively). However, the sample number of pre-frail subjects was too few to be analyzed in despite of the potential for getting better with 12 weeks intervention. Conclusion: The smartphone frailty screening system has a beneficial substitute for traditional manual measurements, as its immediately detection and exactly diagnosis. Besides, the computerized methods provide quantified data to represented the severity of frailty. It will help determination of intervention strategy for frailty syndrome and economized medical expenses and social expenditure.
