The condition of spilled gallstones is rare and occurs in 6% to 30% of laparoscopic cholecystectomy. Although spilled gallstones were considered harmless, unretrieved gallstones may cause uncommon but significant complications. We reported a rare case with subphrenic abscess due to a spilled gallstone 8 years after laparoscopic cholecystectomy. In cases of gallstone spillage during laparoscopic, every effort should be made to retrieve the stones. Once spillage occurred, it must be clearly documented and patient should be informed to promote the early diagnosis of late complications.
