Objective: Understand the different clinical signs of patients who had an appendectomy. Methods: Retrospective review of medical records. Data on blood tests, pathologic reports and admission details were collected. Comparisons were made between patients with appendicitis vs. a normal appendix and those with a perforated vs. non-perforated appendix. Results: The diagnostic accuracy rate for appendicitis was 85.8% in 908 cases. The mean age of the patients was 34.1 years old. The perforation rate was 36.5% and the complication rate was 3.5%. The sensitivity of a diagnosis of appendicitis reached 80% when the marker of a white blood cell (WBC) count over 13000/uL or neutrophil percentage over 88% was used in patients with right lower quadrant pain. C-reactive protein values were higher in the perforated than the non-perforated group. Patients older than 60 years had a higher perforated rate. Conclusions: Although the WBC count and neutrophil percentage were good appendicitis markers, a complete history and detailed physical examination are essential. Image study may be necessary to help diagnose appendicitis correctly.
