We reported a rare case of simultaneous transscaphoid perilunate fracture-dislocation (PLFD) of one wrist and perilunate dislocation (PLD) of the other in a patient with multiple traumatic injuries following traffic accidents. PLDs and PLFDs are uncommon, challenging, generally high energy injuries that carry a guarded prognosis.
