Staged two-level pedicle subtraction osteotomy for severe thoracolumbar kyphosis in ankylosing spondylitis-A case report

Ankylosing spondylitis (AS) is an inflammatory disorder of unknown cause that primarily affects the axial skeleton. Kyphotic deformity is common, and spinal osteotomy is usually mandatory to restore trunk balance. Many spinal osteotomy techniques show good results in mild to moderate kyphosis (< 70°). However, only a little literature focuses on the patients with severe kyphosis (> 70°). We report the case of a 28-year-old man with clinical and radiological diagnosis of AS with severe thoracolumbar kyphosis (110°) who received staged pedicle subtraction osteotomy at two levels. Literature review was performed. This technique is a safe and effective alternative to correct thoracolumbar kyphosis and to restore spine balance.
