Optimal Cut-offs for Overweight: The Association between Body Mass Index and Hypertension in College Students Population

大學生族群體重過重切點:身體質量指數與高血壓之關連性

摘要


本研究目的在於評估大學生族群身體質量指數與高血壓之關連性,並訂定代表高血壓盛行之過重切點。本研究亦以2004至2008參加健康檢查之學生為研究對象,共10,129位。應用操作者特徵曲線(ROC)分析高血壓風險之理想切點。結果顯示,高血壓盛行率隨著身體質量指數的增加而提高。操作者特徵曲線分析顯示,預測高血壓風險之身體質量指數切點男生為24.4kg/平方公尺(敏感性:0.489,明確性:0.770),女生為24.9kg/平方公尺(敏感性:0.342,明確性:0.910)。曲線下面積(AUC)男生為0.677(95% CI: 0.661~0.694),女生為0.644(95% CI: 0.593~0.696)。曲線下面積顯示仍有其它因素影響高血壓風險之預測。本研究結論建議,大學生族群身體質量指數男生24.4kg/平方公尺,女生24.9kg/平方公尺可以作為體重過重之切點,而不宜作為評估高血壓風險之指標。

The purpose of this study was to evaluate the association between body mass index (BMI) and hypertension, and to define the predictive value of BMI for overweight which represents elevated prevalence of hypertension in college students population. The subjects of this study were 10,129 students who underwent a health-check from 2004 to 2008. Receiver operating characteristic (ROC) curve analysis was constructed to determine the best predictive cut-off values for hypertension risk. The prevalence of hypertension was elevated as BMI increased. ROC curve analysis showed maximum sensitivity plus specificity at a BMI valus of 24.4 kg/m^2 in male (sensitivity: 0.489, specificity: 0.770) and 24.9 kg/m^2 in female (sensitivity: 0.342, specificity: 0.910) for hypertension risk. The area under curve (AUC) was 0.677 (95%CI: 0.661~0.694) and 0.644 (95%CI: 0.593~0.696) for male and female students, respectively. The moderate levels of AUC indicate that other factors also contribute to the prediction of hypertension risk. In conclusion, this study suggests BMI values of 24.4 kg/m^2 in male and 24.9 kg/m^2 in female may be more appropriate for defining overweight than screening level for hypertension risk in college students population.
