大量傷病患事故之緊急救護派遣分析:以2014年高雄丙烯氣爆為例

ANALYSIS OF RESPONSE AND DISPATCH OF EMS IN MASS-CASUALTY INCIDENT-TAKE 2014 KAOHSIUNG GAS EXPLOSION EVENT AS AN EXAMPLE

摘要


對比於一般緊急救護勤務(emergency medicine service, EMS),大量傷病患事故(mass casualty incident, MCI)對救災單位、醫療單位絕對是急遽且壓力更大的災害事件。2014年的高雄丙烯氣爆故屬於短期分散型MCI,相較於一般國內外所常見的短期集中型MCI會遇上更多的不確定性。本研究透過高雄丙烯氣爆事故之災例調查,儘可能掌握相關資訊以及參數,最後透過電腦模擬採用AnyLogic 8.7.3版本建置模型進行,並透過其功能項目中的離散事件模擬以及代理人模擬,再透過調整不同參數進行分析,依據模擬結果建議相關單位採取災害應變時之必要作為。除了重視派遣救護車能量比例外,亦須關注後送優先順序分配以及短期分散型MCI路程距離之應對措施。

Mass-casualty Incident (MCI) is absolutely a sudden and more stressful disaster event for the rescue units and medical units than for the general Emergency Medical Service (EMS). The Kaohsiung Gas Explosion Event in 2014 is a short-term distributed MCI. It will encounter more uncertainties for the short-term distributed MCI than the short-term concentrated MCI. Through the investigation of the disaster case of the Kaohsiung Gas Explosion Event, this study tries to grasp the relevant information and parameters as much as possible, and finally, Anylogic 8.7.3 version was adopted to build the model through computer simulation. Using the discrete event simulation and agent-based simulation in its function project, and by adjusting different parameters for analysis, the relevant units are suggested to take necessary measures in response to disasters according to the simulation results. In addition to the energy ratio of dispatching ambulances, attention should also be paid to the priority allocation of evacuation and the response measures of short-term distributed MCI's distance.
