運用品管圈手法降低病房檢體退件率

Using Quality Control Method to Reduce the Inpatient Specimens Rejection Rate

摘要


檢驗數據一直以來都是提供醫師對於病患後續診斷、臨床用藥之重要依據,因此錯誤檢驗報告將影響醫師對於患者治療及病人安全,也間接增加醫療成本,降低醫療品質。本研究目的為改善檢體退件率問題,運用品管手法分析2014年7月份至12月份造成本院檢驗科退件率之要因並以品管七大手法之工具輔助,以戴明循環手法改善各單位採檢及送檢習慣,並舉辦護理科教育訓練。改善方案實施至2015年9月份,平均退件率已下降至0.26%、進步率達35%、目標達成率達70%,執行成效良好。本文希望藉此成果分享,在同儕醫院醫事部門能共同矯正採檢、送檢流程之錯誤程序,以達到降低檢體退件率之成效。期望在此對策推廣下能使醫療同仁達成共識,共同為提昇醫療照護品質而努力。

Laboratory data provide important evidence for doctors to diagnose and treat patients. Hence, incorrect laboratory data will influence the patient safety, increase medical costs, and decrease the medical quality. The aim of this project was to reduce high specimen rejection rates at our hospital. We analyzed the main causes of high rejection rates from Jul 2014 to Dec 2014 using quality control tools. After analyzing the root cause, we used the Deming cycle strategy to improve the collection and transportation of specimens and implemented training programs to improve upon these areas of specimen handling. The project continued until Sep 2015 reducing the average specimen rejection rate to 0.26%. In the final project evaluation, we found a 35% progress rate and 70% rate of achievement, suggesting that the project was run well. By sharing our experiences with other hospitals, we hope we can help rectify the incorrect concepts regarding the handling of specimens and improve specimen collection and transportation procedures. We also hope hospital staffers will reach a consensus regarding the reduction of specimen rejection rate using this quality control method.
