醫院工作人員老化知識、對老人態度與未來從事老人服務工作意願之研究探討-以中部某部立醫院為例

An Exploration of the Knowledge, Attitudes, and Future Intentions to Work with Older People among Hospital Personnel in Hospital, Ministry of Health and Welfare in Central Taiwan

摘要


目的:醫療工作人員的老化知識及對老人態度對老人照護結果產生影響,然而,國內此一議題的研究並不多見。爰此,本研究目的在瞭解醫院工作人員的老化知識及對老人態度,並探討老化知識及對老人態度對醫院工作人員未來從事以老人為主要服務對象工作意願的影響。方法:本研究為一橫斷性研究,以結構式問卷對中部某部立區域教學醫院工作人員進行普查,共發出722份問卷,回收552份有效樣本,有效回收率76.45%。統計分析方法包含描述性統計、雙變項分析及多變項邏輯斯迴歸分析。結果:多變項邏輯斯迴歸結果顯示,年齡(OR=1.04, 95% CI=1.011-1.076)、志工經驗(OR=1.90, 95% CI=1.140-3.156)、接觸經驗(OR=2.20, 95% CI=1.227-3.944)、老人課程(OR=1.90, 95% CI=1.078-3.311)、家人支持(OR=5.56, 95% CI=3.270-9.466)與對老人態度(OR=1.74, 95% CI=1.027-2.943)顯著影響醫療服務人員未來轉換從事與高齡者相關工作的意願。結論:醫院人資部門可於醫院工作人員教育訓練課程中,加強老化知識及對老人態度的相關教育訓練課程,尤其是社會層面的老化知識,另可鼓勵醫院同仁朝「高齡/老人/長照」相關系所進修,藉由強化老化知識以增進對老人態度,期使醫院同仁增加從事老人相關工作服務之意願。

Objectives. Hospital personnel knowledge and attitudes towards the older people are known to affect the health outcomes among the elderly patient population. However, little research has been conducted to understand these important aspects of elderly care in Taiwan. Therefore, this study aims to analyze the association between hospital personnel knowledge and attitudes toward older people and their intentions to work with this patient population in the future. Methods. This cross-sectional study used a self-administered questionnaire adopted from Liu and Yeh et al. to collect data from the hospital personnel working in one central Taiwan regional hospital, Ministry of Health and Welfare. The statistical methods included descriptive and inferential statistics. Multivariate regression was used to explore the effect of knowledge and attitudes toward older people on the future intentions to work with the older people among the hospital personnel. Results. Multivariate logistic regression showed that age (OR=1.04, 95% CI=1.011- 1.076), volunteer experience (OR=1.90, 95% CI=1.140-3.156), frequency of interaction with the older people (OR=2.200, 95% CI=1.227-3.944), geriatric-related courses taken (OR=1.90, 95% CI=1.078-3.311), family support (OR=5.564, 95% CI=3.270-9.466), and attitudes toward older people (OR=1.74, 95% CI=1.027-2.943) were significant positive predictors of the intention of hospital personnel to work with older patients in the future. Conclusion. Based on the findings of this study findings, the hospital human resources department can provide more geriatric-related courses for staff education and training and encourage staff members to return to school for geriatric-related programs.
