Dynamic Changes and Lifetime Scores of Quality of Life in Patients with Different Types of Ischemic Stroke, as Measured by the WHOQOL-BREF

Objective: This study intended to quantify the dynamic changes and lifetime scores of quality of life measures (QOL) among patients with different types of ischemic stroke. Methods: The hospital-based cohort of 10,102 patients with first ischemic stroke during 1995~2007 were classified into four types: large artery atherosclerosis (LAA), lacunar stroke, cardioembolism, and others. The survival functions were determined by linkage with the National Mortality Registry. The WHOQOL-BREF questionnaire was administered to a cross-sectional sample of 748 patients to estimate changes at different times post-stroke. The survival functions were multiplied across the different domains of QOL to obtain the lifetime scores for each group. Multiple regression analyses were conducted to explore the effects of different risk factors on QOL. Results: Patients with LAA were affected the most both in survival and quality of life. Our multiple linear regression found that lacunar stroke patients were affected the least in three domains of the WHOQOL. The above differences in physical and social domain scores disappeared after adjustment for functional disabilities. Conclusion: The lifetime scores of QOL were worst in patients with LAA, indicating a poorer survival, a higher functional disability, and a greater demand for long-term care.
