敘事醫學-人類學的醫學人文之路

Narrative Medicine--An Anthropological Approach to Medical Humanities

摘要


敘事醫學最能結合人文、社會與醫學,找出醫病之間共同交流的世界,增進醫病之間相互了解,讓醫療專業人員在臨床實務融入行為和社會科學的能力,反思醫療執業過程的醫學倫理,這門課因而成為醫學院的核心課程。雖然台灣的敘事醫學,前有美國醫學教育改革路徑的指導,後有教育部醫學人文核心團隊(MEH)的推動,各校的敘事醫學仍必須面對體制結構問題,釐清自身可以開創的價值,重新定位之後,各校開拓的方向不盡相同。本文主要是以人類學家的身分進行敘事醫學的教學反思,反映特定機構內醫學人文教學的實況,探討敘事醫學的教學和體制結構問題,觀察醫學教育改革者能否打破人文與科學的鴻溝,持續進行跨領域的互相理解和溝通合作?

Narrative medicine is considered as the best course to integrating humanities, social sciences and medicine in medical practice, as it helps to find out the common world and increases mutual understanding between doctors and patients. It cultivates health workers the capability of behavioral and social sciences with medicine, and connects to the reflection of medical ethics. With the educational revoluation of medical humanities both in US and Taiwan, narrative medicine has become a burgeoning programme. Eeven though American medical humanities education reform led the approach of narrative medicine and medical educators for humanities (MEH) in the Department of Education advanced it in Taiwan, narrative medicine at medical schools encouter their own structural problems. After reorientation, educators of medical for humanities at each medical school carve out their direction and different value. As an anthropologist working at a medical college for 12 years, I discuss narrative medicine with a special focus on the teaching and reflection, and structural problems which influence teaching, to crave out an anthropological approach to medical humanities. In so doing, I wonder if medical education reformers can break down the gap between humanities and medicine, and continue interdisciplinary understanding, communication, and cooperation in the future?
