Nasal non-Hodgkin's T-cell lymphoma-case Report
摘要
鼻部T細胞非何杰金氏淋巴瘤〈nasal T-cell non-Hodgkin's lymphoma〉是一種罕見的鼻腔惡性腫瘤。臨床症狀表現以鼻塞、出血性鼻分泌物等較為常見；而典型的全身性淋巴瘤症狀，如發燒、夜間盜汗、體重減輕等則較不常見。本科曾經歷一名七十四歲男性病患，就診時主訴鼻塞、流膿涕已2-3個月，經鼻咽纖維內視鏡檢查，發現右側鼻中膈有大範圍的深度潰瘍，左側鼻中膈及鼻腔有不規則灰色壞死性組織堆積，雙側口咽及下咽正常，頸部觸診並無發現任何腫塊。電腦斷層攝影檢查顯示，兩側鼻腔有軟組織密度腫瘤及上頷竇有黏膜增厚的情形。病理切片證實為T細胞非何杰金氏淋巴瘤。因屬罕見且臨床上症狀表現易與慢性鼻竇炎混淆而造成誤診，因此提出報告並加以討論。

Nasal T-cell non-Hodgkin's lymphoma is a rare malignant tumor. The clinical manifestation of Nasal T-cell non-Hodgkin's lymphoma is mainly nasal obstruction and hemorrhagic discharge. But, the presence of systemic symptom of lymphoma, such as fever, night sweating, and body weight loss is unusual. We found a case of 74 year-old male patient with the chief complaint of nasal obstruction and mucopurulent discharge for 2~3 months. The local physical examination showed deep ulcer at right septum and gray necrotic tissue at left septum. Biopsy was performed soon. The pathologic finding proved nasal non-Hodgkin's T-cell lymphoma. Because it is a rare malignant tumor and mimic chronic paranasal sinusitis in clinical manifestation which may lead to misdiagnosis, we report and discuss this case.
