繫帶式足踝護具對行走時踝關節內翻防護效果之探究

Protective Effects of Lace-up Ankle Brace on Induced Ankle Inversion during Walking

摘要


踝關節傷害的預防是許多運動員及教練關心的重要課題,繫帶式護具因可重覆使用、成本較低,且能隨時調整鬆緊度,因此受到許多運動員的採用。然而,過去的研究尚未針對繫帶式護具介入對人體行走時突發之踝關節內翻情形的防護效果進行深入的探討。因此,本研究目的為透過自製步道模擬人體步行時踩踏至30度斜坡凹槽藉以探討穿戴繫帶式護具對步行時足部下肢生物力學參數之影響及對踝關節內翻之防護效果。研究方法:利用十部紅外線攝影機與一臺測力板同步擷取15名大學男性學生在穿戴與未穿戴踝關節護具之情形下,分別於步行中採踏30度內翻斜坡凹槽之運動學及動力學參數,並利用重複量數t考驗之統計方法檢定其差異性(α=.05)。結果:在制動期間,穿戴踝關節護具可降低35.25%的最大踝關節內翻角速度、38.08%踝關節最大背屈角速度以及41.5%的向外分力峰值,但在最大踝關節內翻角度方面則無顯著差異。在推蹬期間,穿戴踝關節護具可降低48.73%踝關節最大外旋角速度。結論:穿戴繫帶式護具雖無法減少踝關節內翻角度,但可減緩踝關節內翻之角速度以及向外之地面反作用力進而避免其韌帶伸展過快及拉力過大而導致傷害。

Prevention of ankle injury is an important issue for athletic and coach. Lace-up ankle brace is one of the most popular ankle support devices for preventing ankle sprains because it costs less, is easier to use and can be retightened as needed. However, the protective effect of lace-up ankle brace on induced ankle inversion during walking is still not clear. The purpose of this study was to investigate the effect of lace-up ankle brace on lower extremity joint during induced ankle inversion while walking. Methods: Fifteen college students participated in this study. Each subject was asked to walk at natural speed over the customized pathway (with 30° tilted force plate setup in the middle of pathway) with and without wearing the lace-up ankle brace. Vicon motion analysis system and a force plate were used to record the 3D kinematic data and kinetic data. Repeat measure t-test was used to compare the differences of parameters between braced and un-braced conditions with an alpha level of .05. Results: wearing ankle brace could decrease 35.25% of the maximum ankle inversed angular velocity, 38.08% of the maximum dorsiflexed angular velocity of ankle jointand 41.5% of lateral ground reaction peak force during braking phase. However, there is no significant difference in ankle inversion angle between two conditions. Wearing ankle brace could also decrease 48.73% of the maximum ankle external rotation angular velocity. Conclusion: although wearing lace-up ankle brace could not reduce the ankle inversion degree, it could decrease the angular velocity of ankle and the lateral ground reaction force to prevent tendon injury by stretching too fast and too hard.
