中年繼親母親家庭支持系統對憂鬱症青少年子女復原研究

摘要

憂鬱症患者的家人需要承擔照顧的責任，可能造成家庭失和、無力感、不安全感，甚至引發了家屬的憂鬱一併發生。本文主要探討繼親家庭陪伴孩子渡過憂鬱之照顧經驗，以了解繼親家庭之支持系統運作情形。本文以Bowen(1978)的家庭系統理論為基礎，探討此個案在繼親家庭中，有利於個案憂鬱症復原之歷程，以及繼親關係之衝突現象。從個案之復原歷程﹕危機期－危機現象及行為表徵、復原期－陪伴與支持系統之建立、新生活適應衝撞期－產生新的生活模式與規範、角色期待與緊張關係，分別討論Bowen自我分化與案主行為之矛盾掙扎現象；此外，以McMaser Model討論家庭在面臨壓力時如何運作，及夫妻關係、繼親關係不確定下，與青少年憂鬱之關聯性。本研究針對繼親家庭建立支持系統之建議可分為3點：1.離婚夫妻敵對關係未修復，導致子女衝突矛盾，適時調整繼親子關係以建立幸福感。2.重新檢視家庭關係脈絡，建立繼親子女之歸屬感。3.繼親家庭擁有接納與包容之氣度，有助於建構良好之互動關係。4.循序漸近建立重組家庭，以增進家庭支持功能。家庭可提高親子關係的滿意程度，以降低復發之機率。

A family who has depression child needs to take on caring responsibility. Caring patients for a period of time may cause family discord, sense of mental powerlessness, sense of insecurity, and even lead to the occurrence of depression.This paper explores the accompanying experience of the stepfamily to understand operational support systems. This paper is based on Bowen’s (1978) family system theory as a basis to discuss this case in the stepfamily during the depression recovery process, as well as the conflict phenomenon. Stepmother made adjustments to take care of their daughter and assisted her to adapt to family life and reduce stress. The patient' recovery process can be divided into: 1. Crisis period; 2. Recovery period; 3. conflict period. In this stepfamily, father and stepmom have the communicative pattern of mutual coordination. The attitudes of the stepmother are to care, accompany, listen, coordinate, and not to criticize. The stepfamily has the features of support, construction, inclusion, and reception. It is helpful for the girl to express her feelings. Families can improve the satisfaction of parent-child relationship in order to reduce the probability of recurrence.
