病友組織社會工作的倫理困境

A Study on Ethics Dilemma of Social Work at Patient Organizations

摘要


在健康與人權被關注的社會下,許多病友組織成立,其對於病友與家屬在疾病適應與資源運用上頗受重視。本研究試圖探討病友組織中的社工服務上所面臨倫理議題與實務困境,以提供病友組織社工倫理議題反思與討論。本研究透過焦點團體法蒐集多個病友組織中社工人員及相關工作者的意見;本研究共辦理5場病友組織的焦點團體,總計訪談24人。本研究發現有四大面向的議題,第一是專業角色議題,包含病友特有文化影響專業界線、病友多重關係不利病友志工管理等。第二係專業風險議題,包括病友挪用資源增加服務道德風險、社工保密原則影響信任關係等。第三,專業能力議題,包括社工一人多工分散專業能量、社工專業不足易遭病友質疑。最後,陌生邊緣議題談到疾病社會汙名衍生服務困境、缺乏專屬福利不利需求滿足。綜上,本研究提出的建議包含病友文化、社工專業、社工倫理、資源網絡和政策倡導等五方面。

In the society where attention has been paid to health and human rights, many patient organizations have been established, and importance has been attached to their assistance in disease adaptation of patients and family and resource application. This study attempts to explore an ethical issue and actual difficulty encountered by social worker service in patients' organizations in order to result in reflection and discussion on social workers' ethical issue in patients' organizations. This study performed focus groups to collect opinions from social workers and relevant workers at many patient organizations. This study performed a total of 5 focus groups at patient organizations and interviewed a total of 24 participants. According to research findings, there are four dimensions of the issue. The first is the issue of the professional role: patient's unique culture destructs professional border and patients' multiple relationships negatively influence social workers' management. The second is the issue of professional risk: patients divert resources which causes moral risk, and social workers' confidential principle might influence trust relationship. The third is the issue of professional competence: some social workers undertake several jobs as individuals and it disperses professional capacity. Social workers tend to be questioned by patients when they lack expertise. Finally, there are the points such as marginal issues, service difficulty caused by notorious society of illness and the lack of exclusive benefits which cannot satisfy the demands. Based on the above, this study proposed suggestions on 5 aspects: (1) patient culture; (2) social work profession; (3) social work ethics; (4) resource networks; (5) policy advocacy.
