以同志大學生因初診斷梅毒感染故至醫院求診時,在社工人員協同下,針對其面臨的困境,輔以衛生教育介入進行會談技巧。藉由質性研究方法進行資料收集與分析,遵守告知同意、不傷害與匿名保密等倫理原則,以立意取樣方式從五家醫院進行觀察並輔以深度訪談7位社工人員與所接觸同志間發生性行為之初診斷梅毒感染的16位大學生為受訪者,共計23人。觀察及訪談來收集資料,歸納有知識缺失、焦慮及傷口疼痛之問題;整體分析:(1)利用衛教措施介入來優先改善身體不適症狀,加強其知識層面缺失;(2)針對因罹病產生的情緒低落、焦慮不安,提供傾聽,陪伴鼓勵告知家屬,取得支持及理解。在共同面對疾病過程中,加強出院準備與衛生教育的介入,提升返家後執行自我防護措施之認知技能,使過程連貫完整,並達到全人衛教為目標。
This study provided a descriptive assessment of how social workers used health education in interview skills to enable homosexual college students preliminarily diagnosed with syphilis to restore their interpersonal conflict, and achieve reciprocity with their family members and social workers. Data were collected through observation, individual interviews, and functional health patterns. Purposive sampling was performed to recruit 16 men who have sex with men homosexual undergraduates preliminarily diagnosed with syphilis and 7 social workers from 5 hospitals. In-depth interviews were conducted under the ethical principles of conformity, non-maleficence, and anonymity. The analysis of the data revealed a lack of knowledge, anxiety, and wound pain in the patients. A nursing intervention was firstly implemented to relieve the physical discomfort of the patients, and health education related to syphilis was provided to enable them to address their lack of knowledge about the disease. Next, attentive listening was given to the patients to facilitate them in overcoming their depression and anxiety over their experiences with the disease, and their family members were encouraged to be supportive and understanding to them. Thus, during their treatment, all the patients were supported by their social workers and family members. The patients were prepared for hospital discharge planning and provided further health education to enhance their self-care practices. In sum, this health education approach improved the patients' physical and psychological wellbeing.
