Rasburicase-induced Methemoglobinemia and Hemolysis in a Patient with Malignant Lymphoma
在一個惡性淋巴瘤的病人使用Rasburicase造成變性血紅素血症與溶血
摘要
目的：報告一個因為rasburicase造成變性血紅素血症(methemoglobinemia)與溶血的病患。病例報告：一個未帶有G6PD酵素缺乏的87歲男性在診斷瀰漫性大B細胞淋巴瘤之後，產生了急性腎衰竭與高尿酸血症，在給予rasburicase之後產生了一個急速的變性血紅素血症與溶血。病患接受了輸血治療後復元，腎功能與高尿酸血症在此病程後也恢復正常。結論：Rasburicase造成的變性血紅素血症與溶血可以發生在沒有G6PD缺乏的病患，且可以單純輸血治療處理。

Objective: To describe a case of rasburicase-induced methemoglobinemia and hemolysis in a patient with malignant lymphoma. Case Report: An 87-year-old man without G6PD deficiency presented with acute renal failure and hyperuricemia after diagnosis of diffuse large B-cell lymphoma. After two doses of rasburicase infusion, an abrupt onset of hemolysis and methemoglobinemia developed. The patient received a blood transfusion and thereafter recovered, and his renal function and hyperuricemia also restabilized following this episode. Conclusions: Rasburicase-induced methemoglobinemia and hemolysis occurs in patients without G6PD deficiency and can be treated by blood transfusion alone.
