應用改良式約束手套於加護病患之成效
The Effectiveness of Modified Restraint Glove in Critical Ill Patients

背景：使用約束物品應秉持以最少的約束，提供最多的安全為原則。目的：旨在探討改良式約束手套之臨床應用成效。方法：採準實驗研究設計，2007年5月1日至11月30日，在某醫學中心加護室，採立意取樣，實驗組接受改良式約束手套，對照組接受傳統式約束手套。研究工具包括：約束觀察紀錄表、病人親友及護理師滿意度問卷。結果：共收案改良式約束手套組27位、傳統式約束手套組26位，前者發生手部潮濕、約束部位水腫、約束部位皮膚勒痕較後者少（p＜.05）；收案病人親友51位，改良式約束手套組26位，對約束之滿意度為3.97±.65分；傳統式約束手套組25位，對約束之滿意度為3.62±.78分；收案護理師39位，平均年齡32.1歲，對改良式約束手套之滿意度為3.22±.58分，對傳統式約束手套之滿意度為1.98±.96分；測血糖及測血氧之護理工時，改良式約束手套均少於傳統式約束手套（p＜.05）。結論／實務應用：改良式約束手套較少產生約束合併症、病人親友及護理師有較高的滿意度、能節省護理工時，值得推廣使用。

Background: The principle of using restraint should uphold minimal constraints to provide maximum security. Purpose: This study was to explore the effectiveness of modified restraint glove. Methods: This quasi-experimental study design was used. Subjects were recruited from critical care unit in medical center, during May 1 to November 30, 2007. Subjects were divided by purposive sampling into an experimental group and a control group. The experimental group received Modified Restraint Glove (MRG), whereas the control group received Traditional Restraint Glove (TRG). Research tools included Restraint Diary, Family Satisfaction of Restraints, and Nurses Satisfaction of Restraint. Results: 1) MRG n=27, TRG n=26; MRG less hands sweet, edema, tight mark than TRG ( p＜.05) MRG Families n=26, TRG Families n=25, The average score of the satisfaction of restraint was 3.97±.65 (MRG Families) and 3.62±.78 (TRG Families), 3) 39 Nurses (32.1±6.1 years old), The average score of the satisfaction of restraint was 3.22±.58 (MRG Nurses) and 1.98±.96 (TRG Nurses), Nursing times of check finger blood sugar and oximeter MRG is saving the nursing times than TRG ( p＜.05). Conclusions: MRG not only improvement on patient's safety but also enhance the satisfaction of patients' families and nurses, and saving the nursing times. MRG is worth popularizing.
