Undescended Parathyroid Gland: A Rare but Potential Cause of Persistent or Recurrent Hyperparathyroidism

Ectopic parathyroid gland is the major cause of persistent or recurrent hyperparathyroidism after parathyroidectomy. Undescended parathyroid gland, though very rare, is a potential cause of surgical failure. Tc-99m sestamibi parathyroid scintigraphy with single photon emission computed tomography/computed tomography (SPECT/CT) is helpful to identify the undescended parathyroid gland. We reported two patients with end stage renal disease and secondary hyperparathyroidism. Persistent and recurrent hyperparathyroidism were noted after parathyroidectomy. Tc-99m sestamibi parathyroid scintigraphy with SPECT/CT images accurately identified undescended parathyroid glands. They received surgical removal of the lesions and the pathological results were parathyroid hyperplasia.
