A 58-year-old man who has pancoast adenocarcinoma of left lung and diabetes was arranged a F-18 FDG PET/CT for staging. Eight units of regular insulin were given subcutaneously 80 minutes before F-18 FDG injection to reduce high blood glucose level (259 mg/ dL). It showed suboptimal imaging of prominent FDG uptake in skeletal and cardiac muscles. Repeated scan one month later with low-carbohydrate diets including last dinner and breakfast 4 hours before F-18 FDG injection revealed a blood glucose level near upper normal limit (119 mg/dL) and an acceptable image quality, which showed a FDG-avid mass in pancoast tumor of left lung and another lesion in left lower lobe lobe that was missed in the previous scan.
