Ebstein's Anomaly of Tricuspid Valves with Wolff-Parkinson-White Syndrome: A Case Study

摘要

Ebstein氏瓣膜異常症合併渥夫帕金森懷特症候群（Wolff-Parkinson-White, WPW），是一罕見的先天性三尖瓣畸形心臟病，臨床上會行心臟電氣燒灼術治療渥夫帕金森懷特症候群，或三尖瓣瓣膜置換或修補手術治療，以改善病人的存活率。本文藉由一位25歲之男性個案，因心悸發作頻繁，在確立診斷為Ebstein氏瓣膜異常症併有渥夫帕金森懷特症候群，接受心臟電氣燒灼術3小時後又復發之實際案例，討論Ebstein氏瓣膜異常症併有渥夫帕金森懷特症候群的相關病生理學、醫療處置與臨床照護，及電氣燒灼術後導致渥夫帕金森懷特症候群又復發之可能機轉。

Ebstein's anomaly with Wolff-Parkinson-White (WPW) syndrome is a rare congenital heart disease with malformation of the tricuspid valve. Considering the high risk for sudden death, the catheter ablation for WPW syndrome or surgical intervention or repair of tricuspid valve replacement is usually suggested to the patient to improve the survival rate. In this report, we present a 25-year-old male who suffered from frequent palpitation was diagnosed with Ebstein's anomaly with Wolff-Parkinson-White (WPW) and underwent a catheter ablation. But, the WPW syndrome was reoccurred in three hours after the catheter ablation. Based on the case, this article discusses the pathophysiology, medical treatment, and clinical care of Ebstein's anomaly with WPW syndrome and the possible mechanism of reoccurrence for WPW syndrome after a successful catheter ablation.
