背景:台灣專科護理師培訓以醫院培訓為主,短期醫院培訓課程是否真能提升專科護理師的角色能力,目前為止無實證可考。目的:探討醫院培訓課程對專科護理師角色能力的效益。方法:本研究為類實驗性研究,收案來源為六所醫院中參與專科護理師培訓課程者,於課程開始前、及課程結束後當日,透過結構式問卷測量其進階角色能力之變化。共發出204份問卷,回收有效問卷為131份,反應率是64.2%。結果:醫院培訓課程對專科護理師整體角色能力有明顯之進步(t = 2.04, p < .05)。特別是在「直接照護」、「實務指導」、「臨床研究」、及「人文關懷」等次量表上,呈現顯著差異(p < .05);但在「領導改革」、「倫理決策」、「專業諮商」、「溝通協調」、與「醫療輔助」等次量表的比較上,並無呈現差異(p > .05)。結論:本研究除發現醫院培訓課程對整體專科護理師角色能力有提升效益外,特別在實務照護、關懷與學術能力上更為顯著;至於領導、倫理決策、諮商溝通與醫療輔助等則期待未來在規劃該培訓課程上能再強化。
Background: The preparation of nurse practitioners in Taiwan is mainly hospital-based training programs. There is not clear how effectiveness of these short-term hospital training programs for developing role competency of nurse practitioners. Purposes: The purpose of the study is to investigate the effectiveness of hospital-based training programs on the role competency of nurse practitioners. Method: This study is in a quasi-experimental design. Trainees of nurse practitioner training programs in six hospitals were potential participants. Data were collected before the training courses and immediately after the completion of the courses by using a structured questionnaire of role competency. The study questionnaire A total of 204 questionnaires were distributed and 131 valid questionnaires were returned, with a response rate of 64.2%. Results: The hospital-based training programs significantly improved overall role competency of the nurse practitioners (t = 2.04, p < .05). There were significant pre-post differences in the following subscales: direct care, practical guidance, clinical research, and cultural competency (p < .05). However, there was no pre-post difference in the following subscales: such as leadership and reform, ethical decision-making, professional consultation, communication and coordination, and medical auxiliary behaviors (p > .05). Conclusion: The study results show that the hospital training programs improved overall role competency of the nurse practitioners, especially on direct care and academic competency. These training programs need to be further strengthening on the developing of leadership, ethical decision-making, consultation, communication, and medical auxiliary behaviors competency.
