一種護眼配方組合物對類固醇引起高眼壓與眼睛病變具改善效益

Assessment of an eye care formulation in the prevention of steroid-induced ocular hypertension and lesion

摘要


類固醇藥物在眼睛上的副作用,會抑制眼睛的細胞免疫功能,可以導致原有微生物感染加重,因此類固醇眼藥水通常會與抗生素合併使用。且類固醇眼藥水會抑制角膜上皮癒合,長期使用可造成高眼壓及白內障等眼部病變。本實驗以大鼠眼睛結膜下注射類固醇(betamethasone)誘發高眼壓模式大鼠評估護眼配方(內含熟地、山桑子、葡萄籽、綠花椰菜苗、金盞花、枸杞與杭菊七種萃取物的複方)對大鼠降眼壓之功效。陽性對照組使用降眼壓的臨床眼藥水(0.15 % Brimoidine tartrate,Alphagan-P),實驗組每日經管餵給予護眼配方50 mg/kg,共計四週。每週測量各組SD大鼠眼壓,實驗組給予護眼配方後,其眼壓與陽性對照組有相仿的下降趨勢,實驗四週後皆恢復至正常眼壓數值(p>0.05)。試驗結束後犧牲大鼠,眼球組織病理切片顯示餵食護眼配方均未發現因高眼壓引起前房、後房及玻璃體液擴張、晶狀體退化、視網膜脫離等現象產生,而陽性對照組不具此改善效果。綜合上述動物試驗結果,護眼配方對類固醇誘導高眼壓大鼠具有顯著降眼壓功效,並能改善高眼壓引起的眼睛病變。

Eye drops containing steroids are used to treat ocular diseases mainly to suppress inflammatory immune responses and to alleviate a variety of inflammation-associated symptoms and sequelae. As steroids may inhibit cellular immune functions in the eye leading to microbial infections, steroid eye drops are typically used in conjunction with antibiotics. Steroid eye drops may also inhibit the healing of corneal epithelial wounds and, if used long term, result in elevated intraocular pressure and cataract. The aim of this study was to assess the protective effect of a formulation containing extracts of Rehmannia glutinosa, Lycium barbarum, Vaccinium myrtillus, Brassica oleracea var. italica, Calendula officinalis, and grape seeds. All rats, except those in the negative control group, were given subconjunctival injection of betamethasone twice daily for 3 weeks to elevate intraocular pressure. Rats in the positive control group were given eye drops containing 0.15 % Brimoidine tartrate (Alphagan-P) to reduce intraocular pressure. The formulation was administered to rats with steroid-induced ocular hypertension orally at 50 mg/kg/day for 4 weeks. Functional parameters such as intraocular pressure, anterior/posterior chamber volume and vitreous humour, lens degeneration, and retinal detachment were assessed weekly. Results showed that intraocular pressure improved in rats given the formulation. At the end of the study, histopathological examination of eyeballs revealed that the formulation prevented or delayed the development of ocular diseases. Based on results of this study, we concluded that this formulation can prevent steroid-induced pathological changes in the eyes.
